Thank you for your interest in working with School Ministries of Trumbull County, a Released Time Bible Education program. This application is for all who wish to work directly with the students in our program. No application will be considered until the application form is fully completed. School Ministries maintains high standards as representatives of our Lord and Savior.

DIRECTIONS:

Fill in ALL requested information in black ink when applying on paper.
All forms need to be filled out completely.
All forms not completed will be returned for more information.
When submitting your application, you only need to do one of the following, download a paper copy in the downloadable link above, or complete the electronic form below and click submit.
Return the original paper copy, completed forms to School Ministries of Trumbull County 4997 Mahoning Ave NW Warren, Ohio 44483.
By submitting online at the bottom of the application, you must click submit to get the application to our office.

RESPONSIBILITY: PART I – Child Abuse

Child abuse is as old as the history of mankind. It has many ugly forms and is a problem of severe magnitude and shocking implications. The spiritual atmosphere that Released Time Bible Education programs attempt to provide may be one of the best deterrents possible. When, however, an instance of child abuse is suspected or reported, our leadership must do everything it can to help those in need as quickly as possible, along with the best spiritual and professional guidelines.

I acknowledge my responsibility to be immediately report any suspicions of child abuse to my Released Time director.

I fully understand that the omission and/or misrepresentation of facts requested by Released Time may be cause for immediate dismissal without prior notice.
I authorize the organization to request and obtain information concerning my previous employment and contact the personal references listed herein and understand that, if other reference checks are to be made, my permission will be requested before additional people are contacted.
I further authorize the RTBE director to request personal clearance from Homeland Security and to contact the Department of State Police Central Records Division of this state, to conduct a criminal history file check by name and identifiers, to determine the existence of any arrest resulting in conviction and to furnish a response to the RTBE Director. If I have not been a resident of this state for seven years, I authorize a federal security check in addition to a state criminal file check.
If accepted for service, I agree to abide by all the rules and regulations of the Released Time Bible Education program.
